We want to take moment to express our appreciation to our extensive network of referring professionals throughout the Greater Philadelphia Region. In the previous 12 months, MANNA received referrals for over 1,300 clients from 159 health care practitioners, in 99 organizations across this area.
The dedication of our referral sources have in providing accurate health information allows us to be able to identify which patients are at acute nutritional risk. They also help us during the recertification process by providing information on the clinical progress that our clients have made during the prior 6 months. We use these health updates to both evaluate the need to continue meals, as well documentation of clinical outcome measurements. These clinical outcomes as well as the quality of life outcomes we collect internally help us to measure our impact on restoration of their nutritional stability and overall health.
The Nutrition and Client Services department will be mailing certificates of appreciation out to our exceptional referral sources that are quick to identify at risk patients and provide comprehensive referral forms and prompt health updates.
MANNA is all about improving health through nourishment, but we can only do that when we know who needs the help! Thanks for all the efforts to get our services to the people who have the greatest need.
